Soul Care was the first home health agency in Illinois to achieve accreditation from AADE to furnish the Diabetes Self Management Education and Training program (DSME/T), which is recognized by Medicare as a reimbursable benefit. We provided multi-language services by highly effective and competent diabetes educators to participants in their homes so that they improve their knowledge, skill and self-care behaviors that ultimately lead to a higher quality of life.
