The Home Health Nurse is responsible for the delivery of coordinated nursing care to patients in their place of residence. Implements and evaluates a comprehensive, goal focused care plan in partnership with the patient, representative (if applicable) and caregivers as developed and approved by the physician.


QUALIFICATIONS

  • Graduate of an accredited school of nursing, Baccalaureate degree preferred
  • Current licensure as a registered nurse in the State of Illinois.
  • Minimum of two years of nursing in an acute care or home care setting preferred
  • CPR certification preferred, not required
  • Current Illinois driver’s license with proof of automobile insurance on personal vehicle for business use or alternate reliable transportation
  • Must be empathetic, non-judgmental, tactful, responsible, adaptable, organized, and committed to the purpose and objectives of the organization
  • Must have excellent communications skills.
  • Must have emotional and mental maturity necessary for establishing and maintaining good working relationships with patients, patient’s families, and Agency personnel.
  • Must understand the team approach and be a supportive team member.

The home health aide is a professional employee who provides personal care and related services in the home under the direction and supervision of the home health nurse. Duties may include reading and recording temperature, blood pressure, pulse and respiration. Assist patient with safe transfer and ambulation. Report any changes in patient condition.

QUALIFICATIONS

  • Licensed Certified Nurse Assistant in the state of Illinois
  • Ability to read, write and verbally report clinical information to Agency staff, patient’s, representative (if applicable) and caregivers.
  • Approved on the IDPH’s Health Care Worker Registry to work as an Home Health Aide
  • CPR certification preferred, not required
  • Current Illinois driver’s license with proof of automobile insurance on personal vehicle for business use or a l t e r n a t e reliable transportation.
  • Good communication skills.
  • Experience in home health care or related health care field preferred.

The In-Home Services Worker is a non-professional employee, trained to provide personal care and related services to clients in their home. Performing personal care tasks that may include bathing, skin care, hair care, nail care, oral hygiene, shaving, dressing, feeding, and toileting.

QUALIFICATIONS

  • Completion of the 9th grade; prefer high school graduate.
  • Ability to communicate well verbally; read and write consistent with job requirements.
  • Approved on the IDPH’s Health Care Worker Registry to work as a In-Home Services Worker.
  • Current Illinois driver’s license with proof of automobile insurance on personal vehicle for business use or a l t e r n a t e reliable transportation.
  • Experience with In-Home Health care or related health care field preferred.

The LTC Nurse provides a one-time thorough (hands-off) assessment to establish qualification for in-home services through their respective insurance carrier. The assessment includes but is not limited to documenting the clients medical history and medications, assess cognitive and mental status, assess the home environment for safety, observe ADL/IADLs, determine current service plan availability.
